The Importance of Patient Information Coordinators (PICs)

Program Director, Dr. Moran Discusses the Invaluable Role PICs Play in Patient Care

One of the most valuable assets to our team, and indeed the Apogee model, is the Patient Information Coordinator. These individuals are the heart and soul of a well-run hospitalist program. In our program, the PICs work dutifully to ensure care is coordinated and transitioned in a way that best serves the needs of the patient.

At Apogee, the Patient Information Coordinators are an extension of the physicians themselves, available to assist with scheduling of follow up tests and appointments, notification and communications to primary care physicians, and a point of contact for the patients in the hospital and after discharge for any issues that may arise. They are the face and the voice of the program, interacting with various hospital departments and client physician offices, as well as introducing patients to the concept of hospitalist medicine by visiting them upon admission.

Patient Information Coordinators provide the administrative support to allow doctors to spend more time in direct patient care, and are a great resource for coordinating that care in an organized fashion at discharge. They even follow up all discharges with a phone call to make sure no additional needs have arisen since the patient returned home and to gather feedback on the patient's experience with our team.

With all that goes into running a hospitalist program, I am so grateful for the role the Patient Care Coordinator plays in handling many of the details. Apogee’s investment in administrative resources is a great example of "what´s best for the patient is best for the practice."
